I’ve seen it advertised to have patients lie in the Trendelenburg position while administering Botox. Supposedly the blood flowing to the face which makes veins more prominent, in turn makes it easier for the surgeon to avoid them. Do you think this position really makes any difference?

I can see how this might make sense, but it just seems a little ridiculous. At Contour Dermatology, we have people sitting up, but slightly reclined to make it comfortable for them. There is always the risk of hitting a blood vessel and bruising, but rarely do we see bruising in the glabellar area. Occasionally, there will be bruising around the eyes, because the number of blood vessels are more numerous and superficial there. Physician experience is the most important thing to look for when considering Botox or other cosmetic treatment.

About three or four months after you’ve had an injection with Botox, your muscles will slowly start functioning again and once the Botox has completely worn off, your wrinkles will be back again. Also, over time you’ll start to lose fat and collagen and elasticity, so you may need to add other treatments to address for future aging. But, no Botox does not make your wrinkles worse than before. Think of Botox as a maintenance treatment that you’ll need every three to four months if you want to keep the wrinkles relaxed.

Following injection with Juvéderm Ultra Plus, the filler is usually fixed in place and held by the surrounding connective tissue. However, aggressive massaging could cause the filler to become dislodged. We tell patients not to massage the area or to get a facial massage for the first 24 hours after the treatment, but migrating typically is not a problem.

Absolutely. New patients who come to us who have had fillers elsewhere often want to know if they can get a touch up on their facial filler. As long as the previous filler was a hyaluronic acid filler, you can have additional filler injected immediately.

Botox is very effective for bunny lines at the sides of the nose and treatment for these types of wrinkles typically doesn’t require much product to relax the muscles that cause them. Anywhere from 5 to 20 units should resolve the problem.

Starting with Botox and fillers earlier can help you to maintain your youthful looks. By not allowing the skin to wrinkle early on, your wrinkles will take longer to show up. There is no need to overdo these on a younger face so that frozen look doesn’t appear.

I have seen patients come in from “spas” and other offices with bad results such as droopy eyelids, over extended lips or unevenness too often. There is also an art to these products and once you’ve done these thousands of times, you know what each patient needs to look their best and not get a frozen or uneven look. Today’s mini-facelift procedure is more natural and what I use in my office to avoid that older method that makes the face look pulled. We’ve had excellent results and often combine it with other fillers and lasers to complete the look.

Years ago, there were only surgeries available to keep people looking younger. Unfortunately the traditional facelifts left people looking very pulled and their eyes looking very hollow. The industry now acknowledges that you need to add volume and relax wrinkles and these are done with Fillers and products like Botox as well as a more relaxed mini-lift and eye lifts.
Most of these products and procedures were developed by Dermatologists and we administer far more procedures than a plastic surgeon does for instance. We have the experience to balance the use of all of these tools to get the most beautiful, natural, youthful results.

I think that I’ve seen some stars that have had too much of some of the treatments out there where their faces are frozen. It’s a sign that whomever did their Botox ® let’s say, didn’t have enough experience to know better where to place it for a more natural look. Dermatologists are the leaders in the field for this more natural approach and we do many more treatments than any other field out there.
Madonna is one of my favorite examples of someone that has the most natural approach to aging out there. She has done a great job of only doing the products and procedures that has kept her young looking and natural.
